This is a combined failure of change leadership, project management, organization’s culture, quality of solution, incentives, lack of interest, process failure, technical knowledge, competition, external conditions, regulation, etc.
Overloading or exploiting your best resource, be it people, equipment or process, leads to inefficiency and higher defects, needless to mention increased risk!
Many organisations are focused on long term vision but loose sight of what challenges they face daily & weekly.
